Medical Release Form
Every player must complete and print a Medical Release Form: Click Here for the Form and give it to the Team Representative (Coach or Team Manager) prior to the first game. The Team Representative must keep these on hand during the course of the end of the tournament.
Tournament Check in:
This will be the easiest tournament check-in! There will be no online or in person check in as we will have everything we need through your registration and roster creation.
We look forward to having the Team Representative (Coach or Team Manager) come to the Skyline tent prior to the first game to confirm your team’s arrival and to receive the Coach gift.
Game Check in:
The roster will be printed on the game card for each game. The referee will do check in from that roster. If you feel that you need to have a printed roster for the weekend, you will go into your account and click on the documents and forms tab and print a PDF roster.
Have all Medical Release forms on hand during the course of the entire tournament.
Each team is guaranteed three games with appropriate rest time given between each game. The purpose of the Skyline Cup is to help prepare for the upcoming season. You will want to register for the age division your team will be in during the Fall 2024 season.
